Education Health and Care Plans
An Education, Health and Care Plan (EHCP) is a legal document for children and young people aged 0 to 25 with Special Educational Needs and Disabilities (SEND), who need more support than their school can provide under usual SEND processes.
The EHCP outlines their educational, health and social care needs, the support required to meet those needs and the outcomes they want to achieve.
